CBCL/4-18, TRF/5-18, YSR (Pre-2001) Forms

NOTE: The CBCL/4-18 is no longer available.

The TRF/5-18 and pre-2001 YSR may still be purchased from ASEBA. These forms will remain available only until supplies run out. All current users of School-Age instruments should plan and execute the transition to the new forms (CBCL/6-18, TRF/6-18, YSR) and software (ADM 4.0) as soon as practical.

The new forms and the old forms cost the same. Please note that pre-2001 TRF and YSR forms are available only to researchers that are doing longitudinal research studies.

All pre-2001 forms can be entered in ADM 4.0 and higher versions. In ADM 4.0, click the "Display Previous Versions" button in the New Form window. In ADM 5.0 and higher versions, select the "Out-of-Date Form Versions" window. Therefore, upgrading your ADM does not necessitate an immediate transition to the new forms.
