iForms Service

The iForms Service is incorporated into your own data collection website or desktop application by a knowledgeable programmer. (See Technical Specifications)

Now your own data collection system can make electronic versions of the CBCL/6-18, TRF, and YSR (ASEBA iForms) available for online completion at any computer.

A separate, ASEBA-maintained web site manages your account and downloads output. You can upload a graphic to iForms Service to provide a header for all respondent screens

iForms Service currently includes respondent entry of CBCL/1½-5, C-TRF, CBCL/6-18, TRF, YSR, ABCL, and ASR forms. Profiles, narratives, and entry reports are available in both U.S. English and international translated formats. iForms is currently available in English, and for school-age forms, it is also available in Latino Spanish and Dutch.

Your data collection system sends information about the child and type of form to iForms Service. Respondents can enter data without personal identifiers

Includes a tool for creating a program for automatically downloading output on a schedule. You can process downloaded output to deliver different types of output to different destinations. For example, you can programmatically append data for an analysis file to your permanent data file and send the profiles to an electronic medical record system
